If you get health coverage through Medicaid expansion, expect an official letter from your state this summer. The rules attached to it take effect January 1, 2027.

Under the One Big Beautiful Bill Act, states with Medicaid expansion are required to notify all affected enrollees about new work requirements before August 31, 2026, according to the Center for Health Care Strategies. Twenty-five states have filed a lawsuit in federal court challenging this narrowing as exceeding Congressional intent - but the case has not yet produced a court order blocking it.

States must notify enrollees by August 31 regardless of how the legal fight resolves. Some states are now facing a situation where their notification letters may need to reference a definition of medical frailty that is actively being disputed in court.
