Medicare's Drug Deductible Limit Rises to $700 in 2027

Medicare prescription drug plans will be allowed to charge a deductible of up to $700 in 2027, compared with a $615 maximum in 2026. That $85 increase is a ceiling on what a plan may require before certain drug coverage begins. It does not mean every person with Part D will pay a $700 deductible; some plans have smaller deductibles or none.

The change creates a concrete comparison for beneficiaries reviewing prescription coverage during the current Medicare enrollment period, which runs through December 7. A plan with a low monthly premium is not automatically the cheapest option once deductibles, copayments and covered medications are taken into account.

Two very different spending limits

Medicare.gov lists a separate $2,400 out-of-pocket threshold for covered Part D drugs in 2027, up from $2,100 in 2026. That threshold is not another deductible. It marks the level at which eligible out-of-pocket spending reaches the catastrophic coverage stage, after which beneficiaries generally do not pay for covered Part D drugs for the remainder of the year.

In between the deductible stage and catastrophic coverage, the beneficiary generally pays the applicable coinsurance or copayment under the plan. Medicare explains that the program ordinarily has three phases: deductible, initial coverage and catastrophic coverage. People receiving certain help with drug costs may have different out-of-pocket obligations.

The $700 deductible is a permitted maximum, not a required charge. A retiree whose plan has no deductible may see no deductible change at all, while someone in a plan with the maximum may reach initial coverage after a larger amount of eligible spending. Medicare notes that the cost of the same prescription can differ by formulary tier and whether the pharmacy participates in the plan’s preferred network.

What to compare before December 7

A useful plan comparison begins with the exact medications a person takes. One plan may list a particular drug on a favorable tier while another has restrictions or a higher copayment. Pharmacy networks also matter. A nearby pharmacy may be preferred by one plan and out of network for another, affecting what the person pays at the counter.

The important comparison is a full year’s projected expense using the patient’s actual medicines. Medicare’s Plan Finder can factor in chosen pharmacies, projected premiums and estimated prescription costs. The current enrollment window ends December 7, allowing many beneficiaries to change Part D or Medicare Advantage coverage for January 1.

Medicare’s online Plan Finder allows beneficiaries to review plan premiums, expected drug spending and coverage details. The annual enrollment period gives eligible people an opportunity to switch Medicare Advantage or prescription drug coverage for the coming year. Coverage terms can change even when a person keeps the same insurer.

The published Medicare guidance also notes that premiums can vary by plan and that higher-income beneficiaries may pay an additional amount. Beneficiaries who went without qualifying prescription coverage after first becoming eligible can face a separate late-enrollment penalty. Those expenses arise from different rules, so a deductible comparison alone cannot capture the total cost of joining or keeping a drug plan.

There is no reason to assume that everyone’s costs will rise by $85 or $300 simply because two maximums changed. The actual impact is determined by plan design, medications and any subsidies. The relevant action now is comparing the 2027 plan documents before the December 7 enrollment deadline.
